Coh-Sync Project

The ‘CoH-Sync’ Project is Health and Well-being Hubs designed to support people in improving their health and wellbeing, specifically in relation to the areas of physical activity, nutrition, smoking cessation, alcohol misuse and mental health. The Project wishes to maximise the opportunities for the Community / Voluntary / 3rd sector to be involved in the development of the Hubs which will deliver activities to tackle the risk factors associated with Long Term / Chronic Conditions. A Community Forum associated with each Hub will guide the implementation of the Project in the locality and the Hub will also be part of a project wide Cross Border Network to share learning across the entire Project.

As part of this project a 6 Week Health and Well-being programme providing health and information packs will be sent out to participants home to help combat rural isolation during the Covid-19 Pandemic Lock-down.
